Who we’re looking for:
A passionate, dedicated, sharp Masters-level therapist who wants to be part of a growing, collaborative team. As a Clinician, you will be provided best in class mental health services that will be delivered via telehealth and in person, specifically working with medically ill patients. This is a hybrid (remote and on-site) position, reporting to our Chief of Integrated Care.
What you’ll be doing
Working as a therapist in a fast-growing, innovative mental-health delivery company to develop services for individuals dealing with cancer – either directly or as a family member.
Providing individual and group psychotherapy, as well as working creatively in the delivery of mental health services.
Collaborating with other Forge Health clinical services as appropriate.
Provide comprehensive assessment and diagnosis using the DSM-V
Provide psychotherapy to individuals and groups in an outpatient setting.
Completion of Initial Assessments for clients, as well as assessing for appropriate level of care.
Collaborate in developing and implementing psychotherapy services for oncology patients and their loved ones.
Obtain and review records relevant to the current treatment episode, and contact relevant referral sources, family or outside systems as needed.
Prepare (as needed) and maintain treatment plan and necessary revisions, in conjunction with the case manager.
Coordinate with discharge planner regarding continuing care and recommendations for discharge.
Documentation of all counseling services in clinical records.
Maintain all necessary logs required for administrative tracking.
Complete relevant state-required work for clients on your caseload.
Participate as a member of a multidisciplinary team assigned to the client, and participate in case management meetings.
Communicate with family members, attorneys, EAPS, doctors or other outside resources, as needed, within provision of consent to release documents, and document all communications.
Initiate releases, contracts for safety or any other necessary documents as needed.
Provide crisis intervention and other functions as needed.
Perform other duties as assigned.
What you’ll need:
Valid, unrestricted license in New Jersey as a LCSW or similar credentials including LPC.
Masters Degree in Social Work or a similar field
At least 2 years of experience as an independently licensed provider
Experience working with individuals and families affected by mental illness.
Experience working with medically ill populations, and preferably (but not necessarily) with cancer patients.
Experience in providing outpatient psychotherapy – preferably evidence-based treatment.
Willingness to engage in the development of new programs and systems of care-delivery
Demonstrates integrity and ethics at all times.
Comfort with discussions of psychiatric medications and medical diagnoses
Trained and experienced in providing psychotherapy in oncology settings (preferred)
Trained in oncology-specific psychotherapies (preferred)
