In 2015, the Quality Institute created the Conversation of Your Life (COYL) program, which provides mayors and community partners with tools to develop a community conversation around end-of-life care planning in a culturally sensitive and accessible way. Now there are 11 counties across the state that convene county task forces including: Bergen, Burlington, Camden, Gloucester, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Somerset, Union.

We are actively recruiting Task Force members for each county to help Garden State residents and their families better understand the benefits of advance care planning and the choices available to them. Use our COYL tri-fold pamphlets in English and Spanish to better understand advance care planning and why it’s important. These patient-friendly documents include a checklist to complete an advance care planning journey, useful terms, useful resources, good books to read, and good films on the topic.
