Join us for a dynamic panel discussion featuring three social workers from diverse areas of aging — hospice and palliative care, community-based practice, and private practice. Panelists will share how Advance Care Planning (ACP) and end-of-life care intersect with their work, the importance of these conversations, and strategies for supporting clients and families. This session will highlight the vital role social workers play in guiding meaningful discussions and ensuring person-centered care across settings.
