One of the important strategies for aging well is to have a plan. We are all familiar with financial planning, but life planning and organization is equally important to ensure that you, your family and your loved ones know what you want and where all your important information is. Whether preparing legal documents like a will, advanced directives and health proxies, or simply getting paperwork organized, setting things up in advance will make it easier for you, your family and friends to help you when needed and focus on what is important.

None of us can predict what will happen, but you can prepare by making sure those who matter most know what you would want if you are ever unable to speak for yourself due to a medical emergency or serious illness.

This session is part of Conversation of Your Life (COYL), a New Jersey Health Care Quality Institute initiative that empowers you to discuss, document, and share their health care priorities with those who matter most. The goal is to make sure your voice is heard if you are unable to speak for yourself due to a medical emergency or serious illness.
