Thirty-Six Hospitals in New Jersey Earn an “A” on the Spring 2025 Leapfrog Hospital Safety Grade
Five More “A” Grades than Last Round; 18 New Jersey Hospitals have Sustained “A” Grades on the Leapfrog Hospital Safety Grade for Five or More Grading Rounds
TRENTON, New Jersey — May 1, 2025 — The Leapfrog Group, an independent national nonprofit focused on patient safety, today released its Spring 2025 Hospital Safety Grade. The biannual Safety Grade is an “A,” “B,” “C,” “D” or “F” assigned to all general hospitals in the United States based on their ability to protect patients from medical errors, accidents, injuries, and infections. Driven in New Jersey by the New Jersey Health Care Quality Institute, the Grades give the community critical insight into their local hospitals.
“Preventable medical errors remain a significant challenge in hospitals across the nation, and far too many people are harmed or killed every year as a result. We are proud of the 36 New Jersey hospitals that earned an “A” and we are proud that 100 percent of our state’s hospitals voluntarily participate in the Leapfrog survey,” said Linda Schwimmer, President and CEO of the Quality Institute. “We will continue to work with our community, our members, and our hospital leaders to make sure patients always come first.”
Here is a breakdown of the Safety Grades in New Jersey:
- 36 A hospitals – 54% (5 more A’s than last round)
- 18 B hospitals – 27% (5 less B’s than last round)
- 12 C hospitals – 18% (1 less C than last round)
- 0 D hospitals – 0% (same as last round)
- 0 F hospitals – 0% (same as last round)
New Jersey ranked #3 in the country for percentage of A hospitals (54.5%), compared to #5 in Fall 2024 (46.3% A’s).
In the Survey, 66 hospitals received a grade. Ten hospitals went up a grade, and seven went down a grade. Three hospitals participated but did not receive a grade. They are:
- Bergen New Bridge Medical Center and CarePoint Health Christ Hospital due to not enough available data.
- Inspira Medical Center Mannington did not receive a grade. As per Leapfrog policies, the facility was issued a new CMS Certification Number (CCN) in 2024 and is therefore treated as a new facility and not eligible for a Safety Grade this round.
For the first time, Leapfrog is spotlighting the 346 hospitals nationwide that not only achieved an “A” for spring 2025 but sustained the grade for two years or more. In New Jersey, 18 hospitals have achieved straight “A’s” since 2023. This select group, representing 12% of all eligible hospitals nationwide, is now highlighted on www.hospitalsafetygrade.org.
To date, New Jersey has the highest rate of Leapfrog Hospital Survey participation in the country at 100%.

The Leapfrog Hospital Safety Grade is the only hospital ratings program focused exclusively on preventing medical errors and patient harm. It is updated biannually in the fall and spring, offering the public a reliable, independent assessment of hospital safety. The Grades are determined by public, peer-reviewed methodology, calculated by top patient safety experts under the guidance of a National Expert Panel. The grading system is fully transparent and always freely available to the public at www.hospitalsafetygrade.org.
