Mayors Wellness Campaign 2020 Healthy Town Application

1. Describe your community’s demographics. If you applied to be a Healthy Town in 2019 and there are no major changes, you may respond “See 2019 Application.”

The MWC recommends completing a local needs assessment to get the facts about your town’s health to ensure that your programming addresses the greatest health needs and challenges in your community. We recognize that due to the pandemic, this process may not have taken place in 2020. Please describe any pandemic related taskforces, committees, or emergency response groups that your municipality participated in and how it helped to inform the decision-making in your municipality.

3. Describe the members of your MWC Committee and how they collaborate to create town programming. Please include whether any residents serve on your MWC Committee and how often your committee meets. If you do not have a MWC committee, please respond “N/A”. (No more than 100 words)

4. The pandemic has shed light on the importance of social determinants of health and their effects on New Jersey residents. There are 5 key areas of social determinants of health (https://www.healthypeople.gov/2020/topics-objectives/topic/social-determinants-of-health). 5. Describe how your MWC programs were promoted and indicate how you may have adjusted your marketing approach due to the pandemic. 6. List and describe up to 10 of the programs offered your MWC during 2020 that you feel have been the most successful. For each program, identify the health goal(s) addressed, if social determinants of health are addressed, the target populations, the sponsoring or partner organizations, the content of the program, the cost, if any, to participate, location (note if event was virtual), attendance, how attendance is monitored, and how programs were evaluated.
